As at 5 Oct 2014, there have been 71 cases (30 confirmed, 26 probable, 15 suspected) of Ebola virus disease (EVD) reported in the Democratic Republic of the Congo, including 8 among health-care workers (HCWs). In total, 43 deaths have been reported, including 8 among HCWs.
816 contacts have now completed 21-day follow-up. Of 305 contacts currently being monitored, all (100 percent) were seen on 5 Oct 2014, the last date for which data has been reported. This outbreak is unrelated to that affecting Guinea, Liberia, Nigeria, Senegal, and Sierra Leone.

Study: Ebola outbreak in DRC not related to West Africa epidemic

The Ebola virus responsible for an outbreak in the Democratic Republic of the Congo (DRC) that began in July [2014] bears similarities to previous outbreaks in equatorial Africa but is distinct from the virus causing the ongoing epidemic in West Africa and has a different zoonotic origin, say results of a study today [15 Oct 2014] in the New England Journal of Medicine.
The DRC outbreak began near the town of Boende in the Equateur province and has remained confined to that province. From 26 Jul through 7 Oct [2014], there have been a total of 69 suspected or confirmed cases, 8 of them among healthcare workers, says the study. Case numbers peaked in the 2nd half of August and have now fallen. 49 deaths have been reported.
The most recent Centers for Disease Control and Prevention (CDC) report on the outbreak, from 9 Oct [2014], says there have been 68 cases and 49 deaths.
The study authors say genome sequencing of samples from DRC patients revealed a local variant of Ebola Zaire as the cause. It showed 99.2 percent identity with the most closely related variant from a large outbreak in Kikwit, southwest of Boende, in 1995. The virus showed 96.8 percent identity to the strain now circulating in West Africa.
The index case was in a pregnant woman who cut up a dead monkey that her husband had discovered. She became ill and died about 2 weeks later. A doctor and 3 health workers who did a postmortem cesarean section on her also became ill and died. Further cases followed.
The authors speculate that among the reasons the current and past DRC Ebola outbreaks have stayed more contained than the West African outbreak is that the former have occurred in more remote areas with fewer human contacts and that the response in the DRC has been rapid, with more experience gained with each one (the DRC has had 6 previous Ebola outbreaks since it was 1st seen there in 1976).

A 3 month Ebola outbreak in the Democratic Republic of Congo has ended after claiming at least 49 lives, the country's health minister says. Felix Kabange said no new cases had been registered since 4 Oct 2014, though he warned against complacency. The country's outbreak is unrelated to the one in West Africa which has claimed more than 5000 lives.
Ebola was 1st detected in 1976 near the Ebola River in what is now the Democratic Republic of Congo. Saturday's [15 Nov 2014] announcement came 42 days after the last new Ebola case in the country. Ebola outbreaks are usually declared over when 2 full cycles of the virus' 21-day incubation period finish without further infections. The outbreak in the Democratic Republic of Congo, which began in August 2014, had a high [case] fatality rate at 74 per cent.
While the epidemic involved a separate strain of Ebola to the one devastating West Africa, Mr Kabange said that his country "remains threatened by the possible import of the Ebola virus disease" from the region. The Ebola outbreak in West Africa is the worst on record. The World Health Organization says 5160 people have been killed, and more than 14 000 have been infected.

Its occurrence in a remote rural area, early recognition of the disease, and prompt institution of infection control measures contained the outbreak within a few generations, in contrast to the unremitting and explosive West Africa situation. While there are subtle genetic differences between the Ebolavirus Zaire strains causing disease in the DRC and West African outbreaks, there is no evidence that biologic differences in the virus account for the very different epidemiologic situations.
With the passage of 2 incubation periods since the last case, the outbreak can now be safely stated to be concluded. Better understanding of the wildlife reservoirs of the virus and education of communities in safe practices in game hunting and consumption may help avoid future introduction of Ebola into human populations. - Mod.LM

Two outbreaks, 2 entirely different outcomes. The World Health Organization has declared an outbreak of Ebola over in the Democratic Republic of Congo after just 66 cases and 49 deaths. It lasted 3 months. Yet the epidemic in Liberia, Sierra Leone and Guinea has been going on for 9 months, with more than 15 000 cases, 5000 deaths and no end in sight.
What's the difference? Experts say experience matters; it was the 7th outbreak in the former Zaire. But equally important is the fact that the village where it started was extremely remote, and the country has a rudimentary system of healthcare workers who know to look out for Ebola.
The latest outbreak started when a pregnant woman was preparing game [hunted wild animals] that her husband had brought home for the family in Ikanamongo, a village in northwestern Democratic Republic of Congo (DRC). After she died of an unknown illness, healthcare workers performed a cesarean section to remove her fetus. "It's local custom; you can't be buried with a fetus inside," said Ben Monroe, a Centers for Disease Control and Prevention [US CDC] epidemiologist who's worked in both the DRC and Liberia. "All the health care workers involved fell ill." Within a week of the unfortunate woman's death on 11 Aug 2014, there were 23 other cases, and 13 of them had died.
"That raised a lot of alarm in the system. Someone in ... the health authority there recognized what was going on," said Monroe. "Congolese people have been hearing about Ebola for years and years and years," Monroe added. "They put a fair amount of effort into preparing for situations like this." Not too far away, the Ebola River flows. Dr. Peter Piot, who discovered the Ebola virus in 1976, named it after this river. [Correction: it was actually named by Karl Johnson's group from the US CDC. - Mod.JW]
It also didn't hurt that the CDC had been studying monkeypox in the same forests since 2010. "We go every year and train health care workers on infection control and disease surveillance," Monroe said. "There's been a lot of public education in this particular region in addition to training health care workers."
Local health experts knew to quickly identify potential cases and isolate them. It doesn't help the patients much -- the death rate is still very high -- but it keeps the virus from spreading. Isolation is also not difficult. The area is very heavily forested, and most people live along the rivers. There's not much travel or trade.
Compare that to the area where Guinea, Liberia and Sierra Leone come together in West Africa. The current boundaries cross over ancient kingdoms, and people travel freely across the borders in trucks, by canoe, on motorcycles and on foot. There's a lot of informal trade and commerce. Years of civil war have left little infrastructure and even less trust in government, especially in Liberia. Ebola had never been seen there. When someone shows up with fever at a clinic, health workers usually assume it's malaria, and if there are more severe symptoms, they might suspect another viral hemorrhagic fever, Lassa fever, or yellow fever. Ebola didn't enter anyone's minds for months as the virus was carried across borders and finally into crowded cities like Monrovia...
Ironically, many of the DRC's Ebola experts were not even on hand for the recent outbreak there. "They were in Guinea," Monroe said. "The DRC actually exports their Ebola expertise to other countries."... - more

Ebola in Kongo DRC - Nähe Bikoro (Provinz Equateur, NW)
In der Demokratischen Republik Kongo scheint wieder das Ebola ausgebrochen zu sein. Es soll bisher 17 Todesopfer geben.

Die örtliche Gesundheitsbehörde hat in den vergangenen Wochen von 21 Fällen von hämorrhagischem - mit Blutungen einhergehendem - Fieber in der Provinz Äquator berichtet. 17 Patienten starben.

Virus erreicht Stadt im Kongo - "Neue Phase des Ebola-Ausbruchs"Der Fall sei in Mbandaka mit fast 1,2 Millionen Einwohnern festgestellt worden, teilte die Weltgesundheitsorganisation (WHO) mit. "Eine neue Phase des Ebola-Ausbruchs ist nun eingetreten", sagte Kalenga. Die zwei zuvor bestätigten Fälle waren in Bikoro etwa 150 Kilometer von Mbandaka entfernt im Nordwesten des Landes festgestellt worden. Bislang gibt es der WHO zufolge 20 wahrscheinliche und 21 mutmaßliche Fälle. Die Regierung tue alles, um den Ausbruch einzudämmen, sagte Kalenga. Inzwischen seien 5400 Dosen eines experimentellen Impfstoffs im Land eingetroffen. Damit sollen vor allem Mitarbeiter in Gesundheitseinrichtungen und Menschen, die mit den Ebola-Patienten in Kontakt gekommen waren, geimpft werden.

Meanwhile, the WHO said on Wednesday it was accelerating efforts with nine countries neighbouring the Democratic Republic of Congo (DRC) to try to prevent the spread of the current Ebola outbreak beyond its borders.

Tote nach neuem Ebola-AusbruchIn der Demokratischen Republik Kongo sind bei einem neuen Ebola-Ausbruch 20 Menschen gestorben. Gesundheitsminister Oly Ilunga Kalenga sprach von "einer neuen Epidemie" der hochansteckenden und oft tödlich verlaufenden Krankheit im Nordosten des Landes. Die Provinz Nord-Kivu habe dem Ministerium 26 Fälle von fieberkranken Menschen gemeldet, von denen nur noch sechs am Leben seien. Von sechs analysierten Blutproben sei das Virus in vier Fällen nachgewiesen worden, erklärte das Gesundheitsministerium. Die Laborproben werden demnach nun weiter untersucht, um die Quelle der Erkrankungen herauszufinden.

Ebola-Ausbruch im Ostkongo - Ein doppelter AlbtraumNoch ist die Millionenstadt Goma im krisengeplagten Ostkongo nicht von Ebola betroffen. Doch überall, in Restaurants, Bars, Schulen, Kirchen oder Büros, werden Vorkehrungen getroffen. Denn sollte sich das tödliche Virus verbreiten, könnte sich die Krankheit aufgrund des regen grenzüberschreitenden Handels rasch in ganz Ostafrika ausbreiten. Das aktuelle Ebola-Epizentrum liegt 240 Kilometer nördlich von Goma, nahe der Millionenstadt Beni. Mittlerweile meldet Kongos Gesundheitsministerium 73 Verdachtsfälle. 46 sind bestätigt, 27 Patienten stehen unter Beobachtung. 43 Menschen sind bereits gestorben [...] Dieser Ausbruch findet unter extrem erschwerten Bedingungen statt. In den betroffenen Provinzen Nord-Kivu und Ituri sind über 100 Rebellengruppen aktiv. Die Armee führt Militäroperationen durch.

Ebola-Ausbruch im Kongo - Checkpoints gegen das VirusRund um die Stadt Beni wurden nun 37 Straßensperren errichtet, die bei Passanten, Fahrern und Passagieren Fieber messen. Knapp zwei Millionen Menschen wurden bereits kontrolliert. Dies soll die Ausbreitung des Virus innerhalb des Landes, aber auch in die Nachbarländer verhindern. Die Grenze zu Uganda ist von Beni nur knapp 70 Kilometer entfernt. Kongos Gesundheitsministerium hat in sechs benachbarten Provinzen Notfall- und Isolationszentren errichtet.

Epidemie im Kongo - Ebola breitet sich rapide ausDer Chef der Weltgesundheitsorganisation (WHO), Tedros Adhanom Ghebreyesus, zeigte sich nach einem Besuch in der betroffenen Stadt Butembo "zutiefst beunruhigt." Die Zahl der Erkrankungen steige wegen Gewalttaten an, "die uns jedes Mal wieder einen Rückschlag versetzen", erklärt Ghebreyesus. Die internationale Gemeinschaft müsse mehr tun, um den Kampf gegen Ebola zu unterstützen. Der Ausbruch in den Provinzen Nord-Kivu und Ituri ist schwer einzudämmen, weil in der Region zahlreiche Milizen aktiv sind. Es gab wiederholt auch Angriffe auf Ebola-Behandlungszentren. Erst vergangene Woche wurde ein Arzt der WHO in Butembo getötet. Nach Angriffen wird der Hilfseinsatz zumeist zeitweise ausgesetzt, um die Sicherheitsmaßnahmen zu verstärken. In dieser Zeit steigt die Zahl der Neuerkrankungen jedoch wieder an.

Über 1000 Tote im Kongo - Ebola praktisch außer KontrolleMilizengewalt nimmt parallel zur Ebola-Ausbreitung rapide zu. [...] Am schwersten betroffen sei der Distrikt Kamango, eine unwegsame Bergwaldregion an der Grenze zu Uganda. Es ist der einzige Distrikt der Gegend, in dem keine Ebola-Bekämpfer aktiv sind, obwohl die Seuche längst dort angekommen sein dürfte. Allein um Kamango seien 60.000 Menschen auf der Flucht – viele davon aus Ebola-Zonen. In der Grundschule des Grenzortes Nobili sind dem Bericht zufolge 7.000 Menschen gestrandet, ohne Versorgung und ohne Wasser außer aus dem Fluss. Der Weg zurück in die Heimat sei von der Armee abgeriegelt, und so versuchten viele, sich illegal nach Uganda durchzuschlagen, was das Risiko einer Ebola-Ausbreitung über Kongos Grenzen hinaus erhöhe.

Hallo alle, eine Frage in die Runde:
war vor kurzem jemand in Uganda oder Ruanda? Hat jemand Infos / Erfahrungen, wie die Ugander mit der Ebola-Bedrohung so nah an ihrer West-Grenze umgehen?
Ich/wir wollen im August in die NP´s im Westen (Murchison- und Queen Elisabeth-NP) und haben natürlich heftige Bedenken...
