Ebola haemorrhagic fever in the Democratic Republic of the Congo - update

Following the confirmed laboratory diagnosis of the presence of the Ebola haemorrhagic fever virus in samples taken from cases in the province of Kasai Occidental of the Democratic Republic of the Congo, WHO is currently upgrading its field operations to support the Ministry of Health in managing and containing the outbreak.

A team from the WHO Country Office has been present in the affected area since 3 September 2007 with further staff from the WHO Regional Office for Africa joining the field operations on 7 September and this week from WHO Headquarters. The WHO team is working closely with representatives of the Ministry of Health and Médecins Sans Frontières to upgrade the existing facilities. Of high priority is the establishment of a mobile field laboratory to be attached to the existing isolation ward in order to expedite rapid diagnosis of patients and to differentiate between the different pathogens, such as Shigella dysenteriae type 1, that have also been associated with this outbreak. Ongoing surveillance and case finding is also being enhanced and social mobilization activities are under-way to provide the local population with all relevant information in order to contain the outbreak.

WHO is also establishing a logistics platform in the vicinity to support the field operations and to facilitate access to the affected area.

Health officials in the Democratic Republic of Congo (DRC) urgently
want to find about 100 people believed to have been close to Ebola
victims and to contain an outbreak that may have taken almost 170
lives, they said Fri [14 Sep 2007]. "We are looking for all people
who may have been in contact with the sick who are presenting
symptoms of Ebola or of shigellosis," Secretary General Benoit
Kebelo, a senior Health Ministry official, told AFP.

The authorities want to stop an epidemic of Ebola, an incurable
haemorrhagic fever that kills between 50 and 90 percent of those
infected, depending on the strain and the speed of intervention to
treat symptoms. "We're looking for around 100 people, especially in
the Kampungu zone," said Kebelo, referring to the epicentre of the
epidemic, some 300 km (185 miles) from the provincial capital of Kananga.

Officials fear that highly contagious Ebola, for which there is no
known cure, may have claimed most of 169 lives out of 376 cases since
April 2007. However, some may be due to the simultaneous appearance
of the _Shigella_ strain of infectious dysentery, which is treatable
by antibiotics. Kebelo said that since the outbreaks were 1st
detected at the end of April 2007, 5 cases of Ebola and one of
Shigella had been formally confirmed by international laboratories.

But for the broader picture, "you must approach these numbers with
caution," warned Kebelo, who heads a joint ministerial task force
coordinating with the UN World Health Organisation (WHO), the US
Centers for Disease Control (CDC) and a team from Doctors Without
Borders (MSF) [Medicins sans Frontieres]. "There are symptoms that
could correspond to other illnesses, and we don't know yet what the
true scope of Ebola or shigellosis is," he said.

The main aim of the government was to prevent an epidemic like that
of 1976, when almost 500 people died in what was then Zaire and [in]
a neighbouring part of Sudan. "Our priority is to raise people's
awareness, stop villagers panicking and fleeing, prevent gatherings
at, for example, funerals and ensure that every suspect case gets to
a health centre immediately," Kebelo said.

The health ministry has deployed "about 100" doctors, nurses,
logistics staff and educators in the danger zone, but these teams
need bicycles to get around the region, the coordinator added.
Belgian MSF teams in the area have been reinforced since late August
2007 after confirmation of the 1st new Ebola case in the country,
where the disease was 1st identified in 1976. The WHO has also sent a
delegation from Geneva with medical supplies and protective clothing
for health staff. "We've set up a quarantine unit in a shed near the
Kampungu health centre," MSF-Belgium coordinator in DRC, Sylvaine
Lonlas, told AFP. "On Thursday night [13 Sep 2007], we had 3 patients
in serious condition and total isolation, and 2 under moderate
surveillance. Six others are convalescing and come back every day for
a check-up."

Ebola's survival rate can be increased by quarantine and swift help
for those infected, who risk high fever, rapid dehydration, internal
haemorrhage and blood loss. The Atlanta-based CDC -- which identified
one of the 1st Ebola cases -- is expected shortly to send an
epidemiologist and set up an on-site laboratory for a more precise
diagnosis of cases, several medical sources told AFP.

The government of Angola, whose northern and northeastern provinces
border Kasai Occidentale, placed health authorities in these regions
on high alert for Ebola on Friday [14 Sep 2007] in response to the
DRC outbreak. After it reappeared in Sudan in 1979, Ebola, which was
named after a river in Equateur province, where it was 1st identified
in 1976, suddenly swept Zaire again in 1995, claiming 245 lives out
of the 315 registered cases.

Ebola haemorrhagic fever in the Democratic Republic of the Congo - update 2

The Ministry of Health (MoH) of the Democratic Republic of the Congo (DRC), with the support of international partners, is continuing field investigations to determine the extent of the outbreak of Ebola haemorrhagic fever in the Province of Kasai Occidental. Active surveillance is under way to investigate recent deaths in the affected communities, to identify other suspected cases and to follow-up on all contacts. Case numbers associated with the outbreak continue to rise and the situation has become further complicated by concomitant reports of cases of typhoid and Shigella dysenteriae type 1.

Isolation wards have been established in the area with the on-going support of a field team from Médecins Sans Frontières (Belgium). Additional isolation wards are also being established in three district hospitals. Epicentre has deployed an epidemiologist to support the MSF team in the field.

Outbreak response field teams are being strengthened and operational bases have been reinforced and established in three towns in the affected area. A central logistics platform is being finalized to provide support to local field communications, to put in place field accommodation facilities and to ensure basic living conditions. The United Nations Department of Peacekeeping Mission in the Democratic Republic of Congo (MONUC) is providing additional logistics support.

Epidemiologists, virologists, laboratory experts and logisticians from the MoH, WHO, the US Centers for Disease Control and Prevention (CDC) and the Public Health Agency of Canada (PHAC) are in the field. Laboratory equipment and outbreak response materials are also being delivered by air with the assistance of MONUC, MSF-chartered flights, and by Interchurch Medical Assistance (IMA World Health), an international non-governmental organization.

MONUC has also assisted WHO in deploying vehicles and other outbreak related response equipment from its Outbreak Logistics Mobility Unit in Dubai. Further shipments of Personal Protective Equipment and specialist communications equipment including satellite phones and radios are also being sent to the country.

Social mobilization activities are being implemented by national field teams with the support of the national Red Cross, the International Federation of Red Cross and Red Crescent Societies and UNICEF. A medical anthropologist has been identified by the Centre National de Recherche Scientifique, Paris to work with the social mobilization teams to develop culturally appropriate information concerning Ebola and to ensure the population is provided with information to reduce the risk of transmission of the disease.

Experts in infection control from the Swiss Agency for Development and Cooperation, the Hôpital Cantonal in Geneva and WHO Headquarters are being deployed to strengthen infection control in the affected area. Precautions are also being put in place health care settings in areas beyond the outbreak zone to reduce the risk of any amplification of the outbreak.

Other partners from the Global Outbreak Alert and Response Network are also providing support to the MoH, including the African Field Epidemiology Network, the Bernard Nocht Institute, the Centre International de Recherches Médicales de Franceville, the European Centre for Disease Control, the Institute Pasteur, the London School of Hygiene and Tropical Medicine, the National University of Singapore, the Swedish Institute for Infectious Disease Control, Training Programs in Epidemiology and Public Health Interventions Network Inc. and Tulane University School of Public Health and Tropical Medicine, USA.

The WHO Country Office in Kinshasa has been strengthened to provide support to the MoH in responding to external requests for information on this outbreak.

Ebola haemorrhagic fever in the Democratic Republic of the Congo - update 4

The Ministry of Health has confirmed that, as of 2 October 2007, a total of 25 out of 76 suspected cases of Ebola haemorrhagic fever from the province of Kasai Occidental have now tested positive for the disease. A further 187 contacts are currently under medical observation and will remain so until each has reached the end of their respective potential incubation periods without developing any symptoms of the disease.

Although there is now strong clinical and epidemiological evidence to suggest that the outbreak is slowing, identifying and isolating all suspect cases is still a priority for the response teams to ensure that the chains of transmission are broken. This includes following up any contacts over the incubation period in order to isolate them immediately if they become ill. Two consecutive incubation periods must elapse, a total of 42 days following the identification and isolation of the last confirmed case, before the outbreak is considered controlled.

The international response team, with the participation of members of the Global Outbreak Alert and Response Network, is continuing to work with the Ministry of Health in all areas of disease control including community health education, social mobilization, contact tracing, clinical management, infection control in health facilities and rapid on-site laboratory diagnosis. The international team is also assisting in conducting retrospective epidemiological studies to further characterize the outbreak.

Outbreak of Ebola haemorrhagic fever in DRC

In mid-September 2007, the Ministry of Health of the Democratic Republic of the Congo (DRC) confirmed an outbreak of Ebola haemorrhagic fever in some remote communities in the province of Kasai Occidental. A team of experts from the ministry and WHO quickly launched a disease control campaign. The team included epidemiologists, virologists, logisticians and experts in social mobilization, infection control, medical anthropology and laboratory diagnosis. The team often had to travel by foot and boats to reach these communities.

Their activities included field investigations, disease surveillance, infection control, clinical management, social mobilization and community health education. Expert support came from partners of the Global Outbreak Alert and Response Network (GOARN) and brought together Médecins Sans Frontières-Belgium (MSF), the International Federation of Red Cross and Red Crescent Societies, and teams from the following countries: Canada (Public Health Agency of Canada, PHAC, Winnipeg), France (Centre National de la Recherche Scientifique, and Natural History Museum, Paris), Gabon (Centre International de Recherche Médicale, Franceville), Switzerland (Swiss Agency for Development and Cooperation (SDC)/ Hôpital Cantonal, Geneva) and the United States (Centers for Disease Control and Prevention, CDC, Atlanta, GA).

Suspected case of Ebola hemorrhagic fever in Boende
---------------------------------------------------
A case of haemorrhagic fever has been reported in the Boende health
zone of Tshuapa District. The chief medical officer in the area, Dr.
Cesar Moyeli, believes that symptoms exhibited by a woman admitted to
a general hospital in the zone are consistent with those of Ebola
haemorrhagic fever.

Dr. Moyeli stated that: "After 2 days, the patient began to
experience haemorrhage: 1st from the genitalia, then from her eyes,
gums and even her ears. In addition red patches appeared on her skin.
Taken together these features suggested a diagnosis of haemorrhagic
fever. In order to avoid disseminating the infection, we informed the
family that, due to the danger of the illness, the body could not be
released to the family for normal burial. Instead the body was
disinfected and taken directly to the cemetery for burial. All
materials used in the care of the patient have been burnt and the
treatment room remains closed."

The patient died last Thursday [29 May 2008]. Clinical samples have
been taken and dispatched to to Kinshasa [the capital of Congo DR]
for virus identification. Meanwhile the family of the deceased and
all her known contacts will be kept under observation for a period of 3 weeks.

Boende: Ebola hemorrhagic fever outbreak suspected

The WHO sub-office based in Mbandaka (Equateur province) reported, on
2 Jun 2008, a worrying trend of reported cases of suspected viral
hemorrhagic fever (VHF) at the General Referral Hospital (HGR) Boende,
in Tshuapa district [see ProMED-mail post Ebola hemorrhagic fever -
Congo DR: susp., corr. 20080604.1789].

Indeed, according to the weekly report of activities "Action Health
Crisis in DR Congo," from 23 May to 13 Jun 2008, 4 suspected cases
were recorded, all fatal. The 4th case, which was followed in
isolation, died on the night of 12-13 Jun 2008, bringing the fatality
rate to 100 percent. The report says that all these cases come from
the health zone of Boende. Of the 4 cases, 2 are in the age group of
less than 5 years, all male, while the 2 others are women 18-25 years old.

The report also stresses that WHO has provided technical, financial,
and logistical support and facilitated the transport of teams of
experts from Kinshasa, and other parts of Mbandaka, capital of
Equateur province, to reach Boende, located 444 km (276 mi) east of
Mbandaka. The report also notes that WHO has provided basic kits and
3 other essential medicines at the hospital to improve Boende's care
for the sick, and personal protection material.

Samples were taken and sent to the CDC (Centers for Disease Control
and Prevention)-Atlanta, USA, lab but the results are not yet known.
Other partners such as UNFPA (United Nations Population Fund),
Medecins sans Frontieres-Belgium (MSF-B), and ECHO (European
Commission Humanitarian Aid) have also given their support to monitor
the suspected viral hemorrhagic fever in Boende.

[On 3 Jun 2008 a single case of suspected Ebola hemorrhagic fever was
recorded in the Boende health zone of Tshuapa district. Confirmation
of the diagnosis has not been received so far.

The article above reports an increase in the number of suspected
cases of Ebola hemorrhagic fever in the Tshuapa district. As of 23
Jun 2008 there have been 4 cases of suspected Ebola hemorrhagic
fever, all fatal. Further information and confirmation of the
diagnoses are awaited.

The 2007 outbreak of Ebola hemorrhagic fever in Congo DR occurred to
the south in Kasai Occidental province; see ProMED-mail references
below.

Ebola haemorrhagic fever in the Democratic Republic of the Congo
WHO - 26.12.2008

The Ministry of Health (MoH) of the Democratic Republic of the Congo declared on 25 December an outbreak of Ebola haemorrhagic fever in Mweka District, Kasai Occidental province based on laboratory results from the Centre International de Recherches Médicales de Franceville (CIRMF) in Gabon. CIRMF confirmed the presence of Ebola virus in 2 samples from 2 of the patients in the outbreak by antigen detection. In addition, laboratory tests conducted by Institut National de Recherches Biologiques (INRB) in Kinshasa, also confirmed the presence of Shigella.

As of 26 December, WHO is aware of 34 suspected cases including 9 deaths (CFR 26%) associated with the ongoing event. Additional samples have been collected and are en route to INRB.

The WHO Country Office, Regional Office and Headquarters are supporting the MoH in Kinshasa and in the field at the location of the outbreak. In addition, the WHO Country Office is supporting the operational costs of the investigation and response teams and has deployed four vehicles to the field. Additional staff, outbreak response equipment and supplies, including Personal Protective Equipment (PPE), are also being sent to the area. An enhanced team of national and international experts is being mobilized to implement control strategies for Ebola haemorrhagic fever and to support outbreak field response in the province.

WHO is working together with laboratory partners to provide comprehensive laboratory services, and to support the MoH in the control and investigation of the outbreak.

WHO is unaware of any reports signaling the international spread of disease and advises against the application of any travel or trade restrictions upon the Democratic Republic of the Congo .

The number of people with suspected Ebola hemorrhagic fever in the
Mweka district of Western Kasai [Kasai Occidental] Province, in the
Democratic Republic of Congo (Congo DR), is now 38, including 12 who
have died. This is the 2nd outbreak of its kind in just over a year
in Western Kasai; the current outbreak claimed its 1st life on 27 Nov
2008.

Last week [ending 27 Dec 2008], international medical humanitarian
organization Doctors Without Borders/Medecins Sans Frontieres (MSF)
sent a team of expert medical and non-medical staff to the area. A
team of 16 are now working around the clock to contain this deadly
epidemic. MSF built a small isolation unit in Kaluamba, the
worst-affected village in Mweka district. Here, to reduce the risk of
the virus spreading, patients who are already sick, along with others
who are suspected of having Ebola, are separated from the community
and cared for. Currently, there are 2 patients in this special ward.
MSF also set up another isolation center further north in the
district, in Kampungu. This center can host up to 25 patients if the
outbreak extends even further.

Ebola is a highly contagious and deadly viral disease. It is
transmitted between people through contact with body fluids (blood,
vomit, diarrhea). Thus, because of their proximity to the patients,
both health staff and family members are at serious risk of
contracting the disease. MSF hired and trained a dozen local "home
visitors" to help reduce the spread of the outbreak. These home
visitors identify and follow up with those people who have had any
contact with the disease. Currently, there are 184 people who must be
tracked. Another 36 local people travel throughout the communities
ensuring that people are properly informed about the disease and are
aware of the health services available to them.

MSF is closely monitoring what could possibly be new pockets of Ebola
in the same district of Mweka. However, other diseases, including
malaria and shigellosis, which can also have similar flu-like
symptoms to Ebola's in the 1st stages, have been confirmed in the
area. Thus, confirmation is still pending from the laboratories to
determine whether these new cases are Ebola or not.

[Since the previous report from the Mweka district on 28 Dec 2008,
the number of fatalities has increased from 11 to 12 and the number
of suspected cases from 21 to 38. Another 184 people are possible
contacts who remain under surveillance. The above report includes a
photograph (which can be accessed via the above URL) of MSF staff in
protective gear working in an isolation unit for patients suspected
of having Ebola in Kaluamba, Western Kasai Province.

Ebola haemorrhagic fever in the Democratic Republic of the Congo - update
WHO - 02.01.2009

The Ministry of Health of the Democratic Republic of the Congo (DRC) is continuing to respond to the ongoing outbreak of Ebola haemorrhagic fever in the Mweka health zone, Province of Kasai Occidental with the support of a wide range of international partners.

As of 31st December there has been a total of 3 laboratory-confirmed cases of Ebola haemorrhagic fever. WHO is aware of 36 additional suspected cases including 12 deaths associated with this outbreak. A further 184 contacts have been identified and are being followed up.

Laboratory analysis was undertaken at the Institut National de Recherches Biomédicales (INRB) in Kinshasa , DRC , the Centre International de Recherches Médicales de Franceville (CIRMF), Gabon, and the National Institute for Communicable Diseases (NICD), South Africa.

The WHO Country Office, Regional Office and Headquarters are supporting the MoH in Kinshasa, in Kananga and in the field at the location of the outbreak. WHO has deployed five vehicles to the field and has sent outbreak response equipment and medical supplies. The local health authorities in the affected area are working closely with social mobilization experts to develop key information messages for the local communities.

The international response to the outbreak includes partners from Caritas (Belgium), the Congolese Red Cross (DRC) , Médecins Sans Frontières (Belgium), UNICEF, the United Nations Organization Mission in the Democratic Republic of the Congo (MONUC), and the World Food Programme (WFP) .
