The causative agent of cholera, _Vibrio cholerae_, has been detected
in stool samples collected from Jajarkot district where more than 150
persons died of diarrhea in recent weeks.
When 5 suspected samples were subjected to laboratory test in
Kathmandu following the Ministry of Health and Population directives,
the cholera causing bacterium was detected in 3.
Though it was believed that cholera was eradicated from Nepal, the
disease broke out in Saptari in 2007. Since then it has been endemic
in the country.
For confirmation, the stool samples are collected and sent to the
National Public Health Laboratory (NPHL) in Teku for laboratory
confirmation.

More than 200 people have died in a diarrhea epidemic in Western
Nepal, the most remote and poorest region of the country, which has
spread unchecked for over 2 months. Rukum, Rolpa, Surkhet, Jajarkot
districts are most affected by the epidemic. In these areas the lack
of adequate roads and hospitals are adding to a rapid spread of the
disease. Dozens of people are infected every day in the villages. In
many cases the bodies of dead lie in the fields contaminating the
waterways. A considerable increase [in the number] of the victims is
expected in the coming months.
Despite the fact that the 1st deaths occurred about 3 months ago, the
government is only now intervening to stop the spread of the epidemic
to the rest of the country. After an initial visit, Mon 20 Jul 2009,
the Prime Minister Madhav Kumar assured the people declaring to have
taken special measures to tackle the epidemic quickly. He
acknowledged the responsibilities of institutions stating that "this
situation is due to a lack of coordination of the competent
authorities."
Meanwhile, the younger population is fleeing from the affected
districts for fear of contagion, leaving the elderly alone in the
villages.

A day after a cholera case was confirmed in the city [Mumbai], 3 more
patients from GT Hospital, CST, were reportedly transferred to
Kasturba Hospital for Infectious Diseases on Tuesday [21 Jul 2009] on
suspicion of the water-borne ailment.
BMC [Mumbai Municipal Corporation] executive health officer Jairaj
Thanekar, however, said, "There are no more positive cases and no
more patients have been transferred."
Meanwhile, civic health officials who had been sticking to their "no
more cholera patients" line admitted that some of the water samples
collected from the Ambedkar. Nagar slum in Colaba were "unfit for
drinking. 3 of the 10 water samples drawn from the area were unfit
for consumption and showed the presence of the _E. coli_ bacteria. 2
of the "unfit" samples were from Shaparji while one was from Dhobi
Ghat [a broad flight of steps leading down to the bank of a river],"
said Thanekar.
The slum had drawn attention after patients landed up in hospitals
with diarrhea and vomiting. One of them, a 35-year-old woman, was
confirmed to be suffering from cholera. Civic officials from the
water department blamed the contamination on the tanker water supply.
On Tuesday [21 Jul 2009], health workers fanned out across the slum
and screened 4000 people in Shaparji and another 1152 in Dhobi ghat,
besides stepping up chlorination in the area.

The mystery fever raging in Sanguem has been confirmed as chikungunya
[virus infection], signalling an outbreak of the disease. Blood
samples of afflicted patients examined by the microbiology department
of the Goa Medical College (GMC) returned positive for chikungunya
[virus infection]. However, no satisfactory explanation was
forthcoming from the directorate of health services (DHS) about the
samples which were negative.
"The laboratory tests have confirmed an outbreak of chikungunya
[virus] as 15 of 30 blood samples tested positive for the disease,"
Dr Dipak Kabadi, deputy director, national vector-borne diseases
control program, told The Times of India on Friday [24 Jul 2009].
The outbreak of a "mystery fever" in parts of Sanguem had been 1st
reported by The Times of India on Tuesday [21 Jul 2009].
Asked about the samples which tested negative, Dr Kabadi failed to
give a convincing reply. "Some samples may have not tested positive
owing to the varying degrees of the presence of antibodies in the
patients, while others may have been suffering from ordinary viral
flu. Nevertheless, the fact that as many as 15 out of 30 samples
tested positive is enough to indicate a chikungunya [virus]
outbreak," Dr Kabadi said.
Explaining further Dr Kabadi said, "It's a self-limiting type of
illness. The symptoms (high fever, swelling of and pain in joints and
body rash) subside after patients are put on paracetamol
[acetaminophen] coupled with hot water fomentation on affected body
parts. People should take care to get rid of mosquito breeding sites
in the area."
Interestingly, the blood sample of private medical practitioner Dr
Avdhut Prabhudesai, showing symptoms of flu, has turned negative for
chikungunya [virus infection].

Die beiden grippeähnlichen, mückenübertragenen Viruskrankheiten sind in Indien verbreitet. Vor allem während der Monsunregen muss mit einem erhöhten Infektionsrisiko gerechnet werden, auch in den Städten. Aus den Bundesstaaten Karnataka (S), Kerala (SW) und Goa (SW) wurden dieses Jahr bereits erhöhte CHIC-Fallzahlen gemeldet. Aus Manipur (NO) und West-Bengalen (NO) werden DEN-Ausbrüche gemeldet. Risiko für Durchfallerkrankungen landesweit. Aus dem Westen des Landes wird ein massiver Ausbruch von akuten wässrigen Durchfällen gemeldet. Cholera wurde in einigen Fällen bestätigt. Die Region gilt als eine der Ärmsten des Landes und wird von Touristen wenig bereist. Typhus kommt im Land verbreitet vor. Seit 2005 wurden jährlich 4 bis 5 Erkrankungen an Polio durch Wildviren mit Herkunft aus Indien registriert, die meisten aus dem südlichen Grenzgebiet. Im letzten Jahr wurden 6 Erkrankungen gemeldet. Die Region gilt als eine der Ärmsten des Landes und wird von Touristen wenig bereist. Typhus kommt im Land verbreitet vor. Seit 2005 wurden jährlich 4 bis 5 Erkrankungen an Polio durch Wildviren mit Herkunft aus Indien registriert, die meisten aus dem südlichen Grenzgebiet. Im letzten Jahr wurden 6 Erkrankungen gemeldet. Lakhs [100s of 1000s] of people in 5 districts of Kerala, especially
the northern regions, are suffering from viral fever mainly caused
[transmitted] by mosquito bite during monsoon season. [A lakh is a

Nearly, 10 people have died so far in these regions. According to the
health reports, more than 1.5 lakh [150 000] people in 5 regions are
affected in just few days and the government is taking all necessary
steps to check the spread of the disease. Special camps have been
arranged and extra doctors have been deputed to these regions. The
severely affected districts of Kerala are Malappuram, Kozhikode,
Wayand, Kannur, and Kasaragod.
Many cases of dengue fever, chikungunya, and hepatitis have also been reported.
The symptoms differ as per the region and the doctors say that this
can be attributed as mosquito borne communicable diseases or viral
fever with severe body pain. Dr Abdul Samad, superintendent of
District Hospital in Malappuram, said "after monsoon the number of
people affected from fever is fast increasing specially in June and
July. Compared to last month [June 2009], a 100 times increase has
been reported; it is not a usual viral fever, the afflicted are
showing symptoms of heavy pain and swelling with rashes. We can
attribute that its chikungunya and like viral diseases, which are
spreading in these regions," he said.
Till 28 Jul [2009], the number of patients had crossed 11 lakh [1 100
000] starting from January this year [2009]. Hundreds of people
afflicted with fever and acute joint pains, belong mostly from hilly
hamlets and tribal areas throng in huge numbers to nearby hospitals
for faster relief. Patients complain of headache and joint pain and
have not experienced such fever earlier.

said in the state assembly Wednesday [29 Jul 2009].
Rane was speaking in response to a calling attention motion moved by
Sanguem legislator Vasudev Gaonkar, who claimed that a chikungunya
outbreak was spreading towards the town from the rural areas of
Quepem and Sanguem, around 60 km [37 mi] from [Panaji]. "The trend
that is evolving is that mining workers are affected by chikungunya
[virus infection]. More than 100 blood samples have been drawn and
have been diagnosed as chikungunya. The outbreak is similar to the
one which occurred in Candolim last year [2008]," Rane said, adding
that the disease first made its appearance in the mining belt of
Quepem and Sanguem in January this year [2009].
"The directorate of health services is monitoring the situation.
Additional staff has also been deployed and health surveys in the
areas have been completed. The patients affected have been
identified," Rane said.
Chikungunya is a virus which is transmitted to humans by
virus-carrying _Aedes_ mosquitoes.

An outbreak of chikungunya has been reported in some parts of
Rameswaram [Tamil Nadu state]. A team of entomologists, who visited
the island a few days ago and collected blood samples from affected
persons, has also confirmed the outbreak. V Uma Maheswarai, deputy
director, Health, Ramanathapuram, told 'The Hindu' on Tuesday [4 Aug
2009] that as many as 55 patients were screened to find out the cause
and types of fever. Most of them tested positive for chikungunya
[virus infection]. One patient tested positive for dengue fever. One
was affected with malaria.
Enormous breeding of mosquitoes in and around Natarajapuram area was
said to be the main cause for the outbreak of chikungunya and other
diseases. Based on the directive of the collector TN Hariharan,
fogging and spraying were done with the support of the Rameswaram Municipality.
A team of doctors and health officials had been asked to closely
monitor the affected areas and patients. The diseases had been
brought under control. There were reports that patients were
improving, officials said. Fogging would be undertaken in nearby
localities, including Verkodu and other fishermen hamlets also, in
order to prevent the spread of diseases. Health officials had been
directed to take preventive steps in all vulnerable areas.
The deputy director said that the mosquitoes, which were causing
[transmitting] chikungunya disease [virus], multiplied in pure water
kept in the open for several days. Those storing water for many days
should properly close the vessels and should not provide any
opportunity for breeding of mosquitoes.

Die Behörden riegelten die Stadt mit 10.000 Einwohnern durch zwei Dutzend Straßensperren von der Außenwelt ab. Aus Angst vor der Seuche versuchen mehrere Einwohner zu Fuß zu flüchten. Mehr als 200 Personen stehen im Krankenhaus unter Quarantäne. Ein 32-jähriger Hirte war das erste Opfer der Lungenpest. Er infizierte sich wahrscheinlich bei seinem Schäferhund, der 3 Tage zuvor verendete und von dem Hirten beerdigt wurde. Man vermutet, dass sich der Hund an einem Murmeltier infizierte, die Übertragung auf den Menschen erfolgte am ehesten durch einen Flohbiss. The town of Aksy [actually Semet village in Asky region] in southern
Kyrgyzstan, along with the surrounding district, has been quarantined
following an anthrax outbreak that has infected at least 5 people,
the country's health ministry said.
The disease spread after meat from an infected cow that had been
slaughtered without a veterinary inspection was sold to locals, the
ministry's press service told RIA Novosti. "According to data from
the national center for dangerous infections, anthrax infection has
been confirmed for 5 residents of the Aksy district following tests,"
the press service said. All of them are currently in a satisfactory
condition. Another 12 patients, who had come into contact with the
infected people, are being examined and are receiving prophylactic treatment.

[This confirms the prior tentative diagnosis for these 5 people. As a
week has passed since that report I think we can assume that this
human outbreak is over except for mainly watch just in case. I am not
sure what it means to quarantine a human Kyrgyz population but I
suspect that the veterinary inspection of the local markets has been
ramped up as well as the medcal clinics being on the lookout for any
further cases. Unless something significant occurs this thread is
cut. Unfortunately this oblast is notorious for anthrax so we can

Residents die of hemorrhagic fever in Tursunzoda District

A total of 3 residents of the Tursunzoda District, some 60 kilometres
[37.3 miles] west of Dushanbe, have died of hemorrhagic fever,
Asia-Plus has learned from Azamjon Mirzoyev, Deputy Health Minister
and Tajikistan Head Sanitary Physician.
The Deputy Minister said that the victims were 2 brothers, aged 50
and 47, from the village of Ziyoratut and the 47-year-old head of the
infectious Diseases Department within the local central district
hospital who died yesterday. According to the Deputy Minister the 1st
case of the disease was reported in the village of Ziyoratut on 27
Jul 2009. "Up to the present, 5 persons have been diagnosed with
hemorrhagic fever and 3 of them died," said. Mirzoyev. "24 persons
that were in contact with those who contracted the disease are
currently under the care of physicians. Antiepidemic measures have
been taken in Ziyoratut." No new cases of hemorrhagic fever have been
reported so far, he added.
The viral hemorrhagic fevers (VHFs) are a diverse group of animal and
human illnesses that are caused viruses classified in 5 different
families of RNA viruses: the _Arenaviridae_, _Filoviridae_,
_Bunyaviridae_, _Togaviridae_, and _Flaviviridae_. All types of VHF
are characterized by fever and bleeding disorders and all can
progress to high fever, shock and death in extreme cases. Some of
the VHF agents cause relatively mild illnesses, such as the
Scandinavian nephropathia epidemica, while others, such as the
African Ebola virus, can cause severe, life-threatening disease.
The _Arenaviridae_ include the viruses responsible for Lassa fever
and Argentine, Bolivian and Venzuelan hemorrhagic fever. The
_Bunyaviridae_ include the members of the genus _Hantavirus_ that
cause hemorrhagic fever with renal syndrome (HFRS), Crimean-Congo
hemorrhagic fever (CCHF) virus classified in the genus _Nairovirus_,
and Rift Valley fever (RVF) virus classified in the genus
_Phlebovirus_. Ebolavirus and marburgvirus are included in the family
_Filoviridae_. The family _Flaviviridae_ includes dengue, yellow
fever, and 2 viruses in the tick-borne encephalitic group Omsk
hemorrhagic fever virus and Kyasanur Forest disease virus.

In Goa wurden dieses Jahr mehr als 2.400 Malaria-Erkrankungen registriert, darunter 570 Fälle der gefährlichen Malaria tropica. In Rajasthan wurden dieses Jahr bereits 7.000 Malaria-Erkrankungen gemeldet, davon 500 Fälle von Malaria tropica. Indian health authorities say they're taking steps to deal with a
widespread outbreak of cerebral malaria in the eastern part of the country.
SK Singh, health officer for India's Munger district, says 6 malaria
victims, including 2 children, have died while under treatment, CNN
reported Sunday [9 Aug 2009].
Singh told the US broadcaster that more than 600 local residents have
tested positive for the _Plasmodium falciparum_ parasite, the presence of
which indicates an elevated risk for malaria. He said fumigation drives
have been launched in Munger district and residents were being urged to
report to health centers if they experience flu-like systems. "We have also
launched an awareness campaign aimed at preventing mosquito breeding and
bites," Singh told CNN.

At least 6 people, including children, have died of cerebral malaria and
more than 1000 were afflicted with the disease in Bihar's Munger district,
a health official said on Sunday [9 Aug 2009].
District health official RN Pandey said the deaths had taken place in
Haveli Kharagpur sub-division of Munger since the last 2 weeks, when the
infections were first reported.
He said nearly 80 villages of Haveli Kharagpur had reported cases of
cerebral malaria.

[The increase in cerebral malaria suggests an increase in resistance. There
was a prior outbreak of cerebral malaria in a different district in Bihar
in 2004. The WHO statistics for Bihar, 2000 through 2006, are as follows

press reports do not mention which drug has been used to treat the
patients. Depending on the drug of choice, resistance is one possibility.
But substandard or counterfeit drugs are a problem in South East Asia and
this needs to be considered every time there is an unexpected treatment
failure (see Malaria drug, adverse events - India: RFI 20090706.2431). - Mod.EP
