Swine influenza and the UK

I haven't been able to read every single ProMED-mail report covering
the new "swine" influenza outbreak but it is possible that the
reports have missed an important point concerning the UK and the rest
of Northern Europe.

I apologise if you or someone have already pointed it out, but for
the time being at least, we should have a breathing space in the
sense that influenza virus epidemics don't normally occur in Northern
Europe during the late spring and summer period.

So it would have to be totally outside precedent if this virus caused
significant infections at this time of the year in the UK.

[Analysis of previous pandemics show that transmission in the
Northern hemisphere stops in the beginning of May. The best analysis
I have found is this paper: Viboud et al. JID 2005;192:233-48.

Even if the present A/H1N1 has pandemic potential it is therefore
highly likely that the outbreak will fade out within the next 2 to 3
weeks, but it will reappear in the autumn.

As pointed out in an earlier posting, the second wave can be more
pathogenic than the first wave, and inclusion of the present virus in
the vaccine for the autumn therefore should have the highest priority
(I am sure it does). - Mod.EP]

Aktuelles zur Schweinegrippe

Mexiko: Aus Mexiko wurden bis zum 26. April mehr als 1.300 Verdachtsfälle von Schweine-Influenza A/H1N1 gemeldet. Es wurden bisher mehr als 80 Todesfälle registriert, wovon in 25 Fällen das Virus nachgewiesen werden konnte. Die meisten Erkrankungsfälle werden aus der Hauptstadt Mexico City gemeldet. Auch San Luis Potosi (400km nördlich) und Mexicali (Grenze zur USA) melden Erkrankungen. Bei Reisen nach Mexiko City sollte man Menschenansammlungen und engen Kontakt zu Erkrankten meiden. Des Weiteren sollten Hygienemaßnahmen wie regelmäßiges Händewaschen und das Tragen eines Mundschutzes praktiziert werden. Der Süden des Landes und die Yucatan-Halbinsel sind bisher nicht betroffen.

USA: In den USA wurden bis zum 27. April 40 Erkrankungen in 5 Bundesstaaten (Kalifornien, Kansas, Texas, Ohio und New York City) registriert. Die Erkrankungen verliefen hier vergleichsweise mild; nur ein immunsupprimierter Patient musste vorübergehend im Krankenhaus behandelt werden. Kontakt zu Erkrankten meiden.

Weitere Länder (u.a. Neuseeland, Israel und Frankreich) melden Verdachtsfälle bei erkrankten Reisenden.
Aus Spanien wurde ein Verdachtfall bestätigt, es handelt sich um einen jungen Mann, der Ende April von einer Mexiko-Reise zurück kam.

Swine influenza - update 4

The situation continues to evolve rapidly. As of 19:15 GMT, 28 April 2009, seven countries have officially reported cases of swine influenza A/H1N1 infection. The United States Government has reported 64 laboratory confirmed human cases, with no deaths. Mexico has reported 26 confirmed human cases of infection including seven deaths.

The following countries have reported laboratory confirmed cases with no deaths - Canada (6), New Zealand (3), the United Kingdom (2), Israel (2) and Spain (2).

Further information on the situation will be available on the WHO website on a regular basis.

WHO advises no restriction of regular travel or closure of borders. It is considered prudent for people who are ill to delay international travel and for people developing symptoms following international travel to seek medical attention, in line with guidance from national authorities.

There is also no risk of infection from this virus from consumption of well-cooked pork and pork products. Individuals are advised to wash hands thoroughly with soap and water on a regular basis and should seek medical attention if they develop any symptoms of influenza-like illness.

Deutschland: In Deutschland gibt es inzwischen drei bestätigte Erkrankungsfälle. Zwei davon in Bayern, einer in Hamburg. Alle drei Infizierten sind kürzlich von einer Mexikoreise zurückgekehrt. In Hamburg handelt es sich um eine 22-jährige Frau, in Bayern werden eine 37-jährige Frau und ein Mann Ende 30 in der Universitätsklinik Regensburg behandelt. Es gibt weitere Verdachtsfälle aus unterschiedlichen Bundesländern. Es halten sich im Schnitt 9.000 Deutsche in Mexiko auf.

Großbritannien: Bei zwei Patienten in Schottland wurde der Erreger nachgewiesen. Sie erkrankten nach einem Mexiko-Aufenthalt an milden, grippeähnlichen Symptomen. Ihr Zustand ist stabil.

Israel: Israel meldet inzwischen den 2. bestätigten Schweine-Influenza-Fall. Zwei kürzlich aus Mexiko zurückgekehrte Urlauber wurden positiv getestet. Ihr Zustand ist stabil. Um den Namen eines unreinen Tiers nicht in den Mund nehmen zu müssen, wurde vom stellvertretenden Gesundheitsminister vorgeschlagen, die Schweine-Influenza in Israel künftig als "Mexikanische Grippe" zu bezeichnen.

Kanada: Kanada meldet bis zum 28. April landesweit 13 Erkrankte. Betroffen sind die Bundesstaaten Ontario (4), Nova Scotia (4), British Columbia (3) und Alberta (2). Die Erkrankungen verlaufen alle unkompliziert. Es gibt weitere Verdachtsfälle.

Mexiko: In Mexiko wurden in den letzten Wochen ca. 2.500 Personen in Krankenhäusern mit Verdacht auf Influenza behandelt. Viele Patienten haben die Krankenhäuser inzwischen wieder gesund verlassen. Da in der Statistik nur die Patienten erfasst werden, die hospitalisiert werden mussten, ist die Dunkelziffer wahrscheinlich um ein Vielfaches höher. Die Mehrzahl der Infektionen werden aus dem Großraum der Hauptstadt Mexiko City und aus den Städten San Luis Potosi (400km nördlich) und Mexicali (an der Grenze zur USA) gemeldet. Die Zahl der möglicherweise auf eine Infektion mit dem Schweineinfluenza-Erreger zurückzuführenden Todesfälle stieg auf 159 an. Jedoch ist die Zahl der labortechnisch bestätigten Infektionen mit 26 weiterhin gering (19 Erkrankungen und 7 Todesfälle). Nachdem die Behörden bereits am Freitag die Schließung von Schulen Universitäten und Museen veranlassten, wurden nun auch Restaurants, Bars, Theater, Kinos, Fitness-Center, etc. geschlossen um eine Ausweitung der Epidemie zu verhindern. Aktuell wird diskutiert den öffentlichen Nahverkehr stillzulegen. Das Auswärtige Amt rät dringend von nicht unbedingt erforderlichen Reisen nach Mexiko ab.

Neuseeland: Insgesamt 14 Personen, unter ihnen 9 Schüler, ein Lehrer und eine Begleitperson des Rangitoto College in Auckland sind nach Rückkehr von einer 3-wöchigen Mexiko Reise erkrankt. Der Erreger wurde inzwischen bei 3 Patienten labortechnisch nachgewiesen. Die Infektion verläuft bei allen Erkrankten mild, sie werden zu Hause mit Oseltamivir behandelt.

Spanien: Zwei Studenten wurden nach ihrer Rückreise aus Mexiko positiv auf das Schweine-Influenza Virus H1N1 getestet. Sie waren Teil einer größeren Reisegruppe. 25 weitere Erkrankte stehen unter Beobachtung.

USA: Die Zahl der Nachgewiesenen Erkrankungen steigt weiter an. Bis zum 28. April meldete die US-Gesundheitsbehörde CDC 64 bestätigte Erkrankungen aus 5 Bundesstaaten. Am stärksten betroffen ist New York City (45 Fälle), gefolgt von Kalifornien (10), Texas (6), Kansas (2) und Ohio (1). Die Zahl der Verdachtsfälle ist deutlich höher. Der Zustand aller Patienten ist stabil und der Krankheitsverlauf ist im Vergleich zu den aus Mexiko geschilderten Fällen relativ mild.

Current level of influenza pandemic alert raised from phase 4 to 5

Based on assessment of all available information and following several expert consultations, Dr Margaret Chan, WHO's Director-General raised the current level of influenza pandemic alert from phase 4 to 5. She stated that all countries should immediately activate their pandemic preparedness plans. At this stage, effective and essential measures include heightened surveillance, early detection and treatment of cases, and infection control in all health facilities.

Da die Seemannslyrik uns gewoehnlich ans Herz greift, gebe ich hier auf dem Wueschi den Anfang eines bekannten Piratensongs zum Besten, wobei ich die individuelle Vertonung fuer die Dusche vorschlage - dort hallt es einfach besser:

The 241 rise in cases from Mexico compared to 23:30GMT of 1 May reflects ongoing testing of previously collected specimens. The United States Government has reported 141 laboratory confirmed human cases, including one death.
