New mosquito type raises concern

Scientists have identified a new type of mosquito. It is a subgroup
of _Anopheles gambiae_, the insect species responsible for most of the
malaria transmission in Africa. Researchers tell Science magazine that
this new mosquito appears to be very susceptible to the parasite that
causes the disease -- which raises concern. The type may have evaded
classification until now because it rests away from human dwellings
where most scientific collections tend to be made.

Dr Michelle Riehle, from the Pasteur Institute in Paris, France, and
colleagues made their discovery in Burkina Faso, where they gathered
mosquitoes from ponds and puddles near villages over a period of 4
years. When they examined these insects in the lab, they found many to
be genetically distinct from any _A. gambiae_ insects previously
recorded.

The team grew generations of the unique subtype in the lab to assess
their susceptibility to the malaria parasite and this revealed them to
be especially vulnerable, more so than indoor-resting insect types.

But Pasteur team-member Dr Ken Vernick cautioned that these
mosquitoes' significance for malaria transmission had yet to be
established. "We are in a zone where we need to do some footwork in
the field to identify a means to capture the wild adults of the
outdoor-resting sub-group," he told BBC News. "Then we can test them
and measure their level of infection with malaria, and then we can put
a number on how much -- if any -- of the actual malaria transmission
this outdoor-resting subgroup is responsible for."

The researchers report that the new subgroup could be quite a recent
development in mosquito evolution and urge further investigation to
understand better the consequences for malaria control.

They also emphasise the need for more diverse collection strategies.
The subtype is likely to have been missed, they say, because of the
widespread practice of collecting mosquitoes for study inside houses.
In one sense this has made sense -- after biting, mosquitoes need to
rest up and if they do this inside dwellings, the confined area will
make them an easier target for trapping. However, the method is also
likely to introduce a bias into the populations under study.

Commenting on the study, Dr Gareth Lycett, a malaria researcher from
the Liverpool School of Tropical Medicine in the UK, said it was an
interesting advance that might have important implications for
tackling malaria. Larvae are collected from pools of water for study.


"To control malaria in an area you need to know what mosquitoes are
passing on the disease in that district, and to do that you need
sampling methods that record all significant disease vectors," Lycett
told BBC News. "You need to determine what they feed on, when and
where, and whether they are infectious. And where non-house-resting
mosquitoes are contributing to disease transmission, devise effective
control methods that will complement bed-net usage and house spraying.
A recent 12M euro [16M USD] multinational project (AvecNET), funded by
the European Union, and led by the Liverpool School of Tropical
Medicine has the specific aims of doing just this."

According to the World Health Organization (WHO), there are more than
200 million cases of malaria worldwide each year, resulting in
hundreds of thousands of deaths, most of them in Africa.

Malaria is caused by plasmodium parasites. The parasites are spread
to people through the bites of infected female anopheles mosquitoes.

2011;331:596-8 ). The study was performed in Burkina Faso, West Africa,
and shows that sampling mosquitoes by collecting of indoor resting
mosquitoes alone fails to estimate the contribution to malaria
transmission by outdoor resting mosquitoes. This is important, as
pointed out in the study, because previous malaria control strategies
using indoor residual spraying with insecticides will not kill outdoor
resting mosquitoes.

By sampling mosquito larvae the study succeeded in identifying a new
subgroup of _Anopheles gambiae_ mosquitoes, which was found to be a
highly effective vector of _Plasmodium falciparum_. The study also
helps to explain why malaria control using impregnated bed nets are
effective where indoor residual spraying has failed in this part of
the world.

Tanzania's southern highlanders have long worried about pneumonia and
other respiratory illnesses brought on by the cool, wet weather. But
as climate change contributes to warmer temperatures in the region,
residents are facing a new health threat: malaria.

In Rungwe, a highland district in the south-western Mbeya region
bordering Malawi and Zambia, malaria is fast replacing coughs, fever
and pneumonia as the most serious local health problem.

The change has taken by surprise the region's residents, who live
over 1000 metres (3200 feet) above sea level and outside Tanzania's
traditional malarial zones. A 32-year-old woman of Isebe village had
no idea she had contracted malaria when she was sent to Makandana
District Hospital in late December 2010.

In 2009, health centres in Rungwe district reported 100 966 malaria
cases, a jump of 25 percent from 2006, hospital records show.

Malaria is now the biggest public health threat facing Rungwe
district, which lies about 940 km (590 miles) southwest of Dar es
Salaam, according to the Tukuyu Medical Research Centre, part of the
National Institute for Medical Research.

1/3rd of outpatients visiting the hospital were diagnosed with the
mosquito-borne illness in 2007, according to records from that year,
making it the most common disease for outpatients. Most highland areas
in the east African country are experiencing a growing burden of
malaria cases, officials at the Tukuya Centre said.

Climatic changes brought on in part by local environmental
degradation are contributing to the growing prevalence of malaria in
the district, said Gideon Ndawala, Rungwe district's malaria
coordinator. "People have cleared the forests, rain has decreased,
temperatures have risen," Ndawala said in an interview.

"(When) I 1st reported on the district in 1983, it was very cold, and
it rained throughout the year except from mid-September to early
November. The weather was not favourable for mosquito breeding," he
said.

Now, however, temperatures are higher and rain more erratic, he said,
and mosquito populations -- which thrive on warmer temperatures and
breed in pools of stagnant water -- are on the rise.

Worst hit by the surge in malaria are Tukuyu district town, Ikuti,
Rungwe Mission and Ilolo, according to district health officials. Half
a century ago, these traditionally cool areas saw no mosquitoes and
did not register any malaria cases, but now the weather is warmer,
said Ambakisye Joram Mwakatobe, a 76-year-old man from Bulyaga village
in Rungwe.

"In the past, we never saw mosquito nets here. I saw a net for the
1st time at the age of 20, when I joined Butimba Teachers College in
1957," he said in an interview at his village home.

Mwakatobe said cases of malaria began to appear several decades ago,
but residents did not relate them to warming temperatures, believing
the mosquitoes instead were arriving on buses from lower regions.

"It was in the 1970s when we started getting malaria here. I thought
it was the buses from Kyela and Usangu that brought mosquitoes," he
admitted. But "the weather also started to change in those years," he
said.

A half-century ago, "it was very cold here, and it rained throughout
the year. Three things were compulsory: a sweater, pullover or heavy
jacket; an umbrella or raincoat; and gumboots," he added. "There was
frost all day long, and cars had to put their lights on. "But today,
things have changed," he said. "Look, now we even put on light shirts.
There is no need for sweaters, gumboots or umbrellas."

Scientists agree that the changing weather is feeding into Rungwe's
worsening malaria problem. "Up until 1960, districts like Rungwe,
Mbeya, Mufindi, Njombe, Makete and Iringa in the southern highland
regions were malaria free. Today is quite different; malaria
prevalence is high," said Akili Kalinga, a research scientist at
Tukuyu Medical Research Centre.

Malaria accounts for 30 percent of the burden of disease in Tanzania
and is a huge drain on productivity, according to a report produced by
research scientists for the Sixth Africa Malaria Day in 2006.

In response to the rising malaria caseload, the government is taking
steps to stem the disease's expansion. Measures include public health
education in newly vulnerable districts on home cleanliness and water
storage, how to eliminate the places of still water where mosquitoes
live and breed, and the use of mosquito nets and fumigation, said Dr.
Sungwa Ndagabwene, Rungwe's medical officer. "The government is taking
serious measures to fight malaria. We started with a 'mosquito nets
for all' campaign, saying every person should sleep under bed nets,"
Ndagabwene said.

The government also has begun spraying the inside of homes with
insecticide, 1st in the Kagera region and now throughout the Lake
zone, near Lake Victoria, he said. It plans to expand the spraying
program, which has helped cut malaria transmission in Zanzibar, to the
rest of the Tanzania's malaria-affected regions.

Such spraying programs aim to kill mosquitoes that land on the inside
walls of homes. Spraying can protect homes for between 4-10 months
depending on the insecticide, according to the World Health
Organisation (WHO).

WHO has approved 12 insecticides it considers safe for such spraying
programs, including DDT, a controversial endocrine disruptor that has
proved one of the most effective ways to control mosquito populations
but that has also been linked to environmental damage and health
problems, including cancer. Ndagabwene said spraying the chemical only
indoors limited its environmental impact. WHO officials have said they
believe the benefits of using the pesticide outweigh its risks.

The Stockholm Convention bans the use of DDT but exempts countries
that choose to use the chemical to control malaria. Tanzania is one of
the world's worst malaria-affected countries, recording 14 to 18
million clinical cases annually and 60 000 deaths, 80 percent of them
in children under 5 years old, according to a 2010 malaria reduction
plan put together by U.S. AID. Children under 5 and pregnant women are
most affected by the disease, Tanzanian health figures show.

Monkeys provide malaria reservoir for human disease in Southeast Asia

Monkeys infected with an emerging malaria strain are providing a
reservoir for human disease in Southeast Asia, according to research
published today [8 Apr 2011]. The Wellcome Trust funded study confirms
that the species has not yet adapted to humans and that monkeys are
the main source of infection.

There are 5 species of malaria parasite that are known to cause
disease in humans, of which _Plasmodium knowlesi_ is the most recently
identified. Previously thought to only infect monkeys, researchers
have shown that human _P. knowlesi_ infections are widely distributed
in Southeast Asia and that it is a significant cause of malaria in
Malaysian Borneo.

Until now, it was not clear whether the infection is transmitted from
person to person, or is passed over from infected monkeys.
Researchers led by Professor Balbir Singh at the Malaria Research
Centre, Universiti Malaysia Sarawak, collaborating with Sarawak State
Health Department, St George's University of London and the London
School of Hygiene and Tropical Medicine, examined blood samples from
108 wild macaques from different locations around the Sarawak division
of Malaysian Borneo.

Their results reveal that 78 percent were infected with the _P.
knowlesi_ species of malaria parasite, and many were infected with one
or more of 4 other species of monkey malaria parasites that have not
yet been found in humans. By comparing the molecular identity of the
parasites from monkeys and those isolated from patients with knowlesi
malaria, the team were able to build a picture of the evolutionary
history of the parasite and its preferred host.

Their analysis reveals that transmission of the knowlesi species is
more common amongst wild monkeys than from monkeys to humans, and that
monkeys remain the dominant host. "Our findings strongly indicate that
_P. knowlesi_ is a zoonosis in this area, that is to say it is passed
by mosquitoes from infected monkeys to humans, with monkeys acting as
a reservoir host," explains Professor Singh.

"However, with deforestation threatening the monkeys' habitat and
increases in the human population, it's easy to see how this species
of malaria could switch to humans as the preferred host. This would
also hamper current efforts aimed at eliminating malaria."

Based on the molecular data, the researchers estimate that the
knowlesi malaria species evolved from its ancestral species between 98
000 and 478 000 years ago. This predates human settlement in the area,
meaning that monkeys are mostly likely to have been the initial host
for the parasite when the species 1st emerged. This estimate also
indicates that the species is as old as, or older than, the 2 most
common human malaria parasites, _P. falciparum_ and _P. vivax_. The
study is published today in the journal PLoS Pathogens.

The authors surveyed 108 wild monkeys and found most were infected
with malaria parasites, including _P. knowlesi_. The study found that
the number of _P. knowlesi_ genotypes per infection was much higher in
monkeys than humans, some genotypes were shared between the 2 hosts
and no major types were associated exclusively with either host.

Over the past few years scattered reports of human _P. knowlesi_
infections have appeared from Indonesia (Borneo) Malaysia (Borneo),
Thailand, Viet Nam and the Philippines.

_Plasmodium knowlesi_ in humans: a comment

The recent paper of Kim-Sung Lee et al (PLoS Pathogens 7(4): e1002015
[available at

comes as welcome epidemiological news helping to clarify that _P.
knowlesi infection_ in humans remains (at least for the time being) a
true zoonosis.

This counters the oft referenced notion published over 3 years ago
describing _P. knowlesi_ as the "5th" human malaria parasite (for
example, NJ White, Clin Inf Dis 2008: 46(2): 172-3 [available at
<http://cid.oxfordjournals.org/content/46/2/172.long>]).

At the same time in a ProMED-mail comment posted on 7 Jan 2008
concerning a ProMED Digest dispatch ('Malaria, P. knowlesi, Human -
Malaysia (02): Sarawak' archive no 20080107.0083), the CID
pronouncement was viewed cautiously until further evidence emerged.

Rather, as suspected then, the infection seen in humans should be
regarded as a legitimate zoonosis until proven otherwise.

In that same comment piece, it was stated that in order to establish
the existence of a natural anthroponotic infection cycle (that is,
'human to human' via a mosquito vector, typically), the area of active
transmission must exclude the presence of simian reservoirs (syn- and
exo-anthropic infected macaques) to definitively conclude this
parasite as a 'true' and independent human _Plasmodium_ species.

Knowlesi malaria as a human disease adaptation is certainly well
within possibility, however the search for irrefutable evidence of
such transmission dynamics must continue.

Cases of malaria among UK travellers rise by 30 percent

The number of malaria infections recorded among UK residents has
increased by nearly 30 percent over the past 2 years. New figures from
the Health Protection Agency (HPA) reveal there were 1761 new cases in
2010. Over the past decade most infections have occurred among people
who visited West Africa or South Asia.

***The HPA is warning travellers to heed advice on how to avoid
malaria, which is the world's 2nd biggest killer.

In 2008 there were 1370 new cases but the following year the numbers
increased to 1495. In 2010, almost 40 percent of UK residents who
contracted the disease had visited either Nigeria or Ghana, while 11
percent had been to India.

The HPA believes these travellers may not have sought or were unable
to access advice on malaria prevention or had not thought they were at
risk because they knew the area they were travelling to. And these
kinds of travellers appear to be more at risk because they generally
stay for longer than other visitors, such as those on business. "It
is a myth that people who have had malaria will not get it again" says
Dr Jane Jones.

They also tend to stay with friends and family rather than in hotels
or resorts, and so are exposed to the same risk of contracting the
disease as local people.

Professor Peter Chiodini, who heads the HPA's malaria reference
laboratory, said the figures - released on World Malaria Day - are a
timely reminder for travellers to take precautions against the
disease.

"Anyone who is travelling to a country where malaria is present
should take travel advice and appropriate medication. Even people
living in Britain visiting the country in which they were born or grew
up, or have previously visited, are not immune from malaria and should
take precautions."

Malaria is a devastating disease in the developing world, accounting
for 20 percent of childhood deaths in Africa. Malaria is spread by
mosquitoes in tropical areas but it cannot be transmitted directly
from person to person. The symptoms include a flu-like illness, fever,
shaking, headache, muscle aches and tiredness, as well as nausea,
vomiting, and diarrhoea.

Dr Jane Jones, head of the HPA's travel and migrant health section,
says while malaria is a potentially deadly disease, it is also one
that is almost completely preventable. "Anyone who is planning to
travel to a tropical destination should always seek advice from their
GP or travel health clinic before their trip. It is a myth that people
who have had malaria will not get it again. Our advice is the same for
all travelers: you must take anti-mosquito precautions and medication
to keep safe."

showing an overall increase in imported malaria cases, but the number
of cases equals the number in 2006 (1758) and it is still lower than
any year from 1993-2002.

_Plasmodium falciparum_ malaria was imported mainly from Ghana and
Nigeria, which are highly endemic countries. This primarily reflects
that persons from Ghana and Nigeria living the UK often travel without
realizing the need for malaria prophylaxis.

The increase in malaria from India is surprising. Recent reviews have
suggested that visitors to India should not be prescribed malaria
prophylaxis. In "Malaria prophylaxis policy for travellers from Europe
to the Indian Subcontinent" (RH Behrens et al. Malar J. 2006 Feb 1;
5:7; <http://www.ncbi.nlm.nih.gov/pmc/articles/PMC1373639/>), the
authors concluded that non-selective prescribing of chemoprophylaxis
for visitors to India, Pakistan, Bangladesh, and Sri Lanka should be
dropped. There have been several postings in ProMED-mail on both _P.
falciparum_ and _P. vivax_ malaria in India. The present UK data could
be caused by changing travel patterns but could also reflect an
